The Health Office for Pre-K through 12th grade is located in the Elementary Office area.
Nurse Andrea and Tessa are ready for another year working to keep the students safe, healthy, and engaged in the classroom. Please look below for all medication forms, immunization forms, and a release of information. See below for some simple guidelines on when to keep your student home.
If your child has any changes in their health status, please let the Health Office know as soon as possible so we know how to better help your child if the need arises. It is important that the "Health Information Form" be completed thoroughly and returned to the Health Office at the beginning of the school year.
